Note: If you have any problem or need to understand more after filling online application for an electronic vaccination certificate with your QR Code, you are requested to contact the local level (metropolitan / sub-metropolitan / municipality / village) where you have completed the full dose of vaccine.

  1. If the name of the municipality, the name of the vaccination center, and the seal are not clearly understood on the vaccine card of Covid-19, the QR-code will not be verified. If you have such a vaccine card, please contact the immunization center where you have been vaccinated immediately.
  2. If anyone is found to have uploaded fake documents, strict action will be taken according to the prevailing law of the Government of Nepal.
  3. You should submit your passport size photo in a simple background where both ears should be visible and the size of the photo should not be more than 250 KB.

(1) Who is this form for?

  • This voluntary registration form is for people over 12 years of age who have not been vaccinated against COVID-19 but are willing to be vaccinated.

(2)What details are required to fill out this form?

  • Full name, full address, date of birth, occupation, health status, mobile number, and identity card number (citizenship, national identity card, driver's license, voter identity card, passport, disability identity card, birth registration).
  • When entering your full address, enter your current address.
  • Write only after making sure that your identity card number is correct.
  • Compulsory details must be filled in the form.

Important Information

Veterans who have been vaccinated at the following local levels who have conducted online vaccinations do not need to fill out an application form to obtain a certified certificate with a QR-Code stating that they have received the full dose of the COVID-19 vaccine. If the name of the municipality, the name of the vaccination center, and the seal of the concerned office are not clearly understood on the vaccine card of Kovid-19, the QR-Code certificate will not be validated.

If anyone is found to have uploaded fake documents, action will be taken according to the prevailing law of the Government of Nepal.
